Primary care doctors have a very limited arsenal as well. They typically resort to outdated, ineffective measures to bring their patients relief in the form of expensive, prescription strength hydrocortisone. When this fails, a referral to a surgeon comes next, but most patients prefer to live in misery rather than get surgery.
